FAB Media Release
(FAB Media Release) I, hereby give FindABaller, Inc. permission to use my name and/or image, narrative, testimonial, or personal story. (Media) They may be used in any medium for any educational, promotional, advertising, internal communications, fund-raising, grant application, or any other purposes that support the mission of FindABaller, Inc.
I understand the following terms:
- The FAB Media release does not guarantee publication of my name and/or likeness.
- My name and/or likeness may appear in the media.
- I may request to discontinue future use and publication of my name and/or likeness, narrative, testimonial, or personal story by submitting a dated request, in writing, to FindaBaller,Inc. at the e-mail address below. I understand that this request will NOT apply to materials published, printed or distributed prior to the dated request.
If the undersigned is below the age of 18, a parent or guardian must sign that they agree to the above.
